The difference between whether you get acne mildly or severely is largely due to hormones, although regular washing with water and a simple soap is taken as a bare minimum.
Other things that affect it are stress and tiredness, using hairgel/wax (which runs down the face during the day), smoking, and eating greasy food with your hands (not the act of eating greasy food in itself).
Eating food with your hands (especially greasy fast/street food) and not washing them afterwards does have a big impact - as it inevitably gets transferred to your face.

at age 11. Thankfully, my mom soon took me to a dermatologist and I was put on a regimen of topical and oral acne medication. I never went on Accutane, though. The various meds helped a lot but even to this day, I'll get a few bad spots once or twice a month.
