Tony is killed. As repulsive as the desire to 'know' what happened is.
David Chase (relatively recently): 'if you look at the last scene really carefully, it's all there'.
Why look carefully if Tony is still alive? It could just be Chase feeding the fire, but...

Character playing the suspicious 'man in members only jacket' is 'Paolo Colandrea', a man with no previous acting experience, who got a call from Chase and claims to have signed papers preventing him from revealing what happened. The point being that the part and actors name together make an anagram of 'A man kills Tony, Carmela, AJ - don become prone', the Don being Tony, prone meaning, literally, lying on your front.
