I'm a fan of Alan Dubin.

I like Stephen Malkmus. He has a great scream, I'm not even joking. Listen to the end of Unfair and Unseen Power Of The Picket Fence.
Steve Snere from TAAS too. He doesn't scream often, but when he does, Jesus, it's brilliant.
Jacob Bannon's a king too. But Jordan and Johnny from The Blood Brothers are my faves. They're just demented.
Best scream = Half a Canyon.
He has a pretty crazy scream, albeit slightly more contained on 'Worship and Tribute'.

I played it in a dj set once and three full on metallers came up to ask what it was.

I think Caleb Scofield is great too. On Trepanning he is the definition of "gutteral." Keith Buckley and Daryl Palumbo are also awesome lyricists too (especially Keith) which makes their delivery even better.
And Maynard James Keenan deserves a mention just for THAT bit in The Grudge.
His screaming on that is incredible.
Caleb sounds like he's shredding his throat / Stephen actually was

pretty much wins it for me. Just for the range and dementedness of his voice.
Though a special mention for Drew Speziale of Circle Takes the Square half way through 'In the Nervous Light of Sunday' for the most alarming full on scream ever. Like a boot stamping on a dog's leg, forever.

and after Saturday's performance by the Neubs, I'd have to add Blixa Bargeld. There were a couple of high-pitched screams that I initially thought were the sound of someone taking an angle-grinder to a steel filing cabinet.

Buried Inside. Sounds like he's getting an electric shock everytime he opens his mouth.
I like the vocal effect on the first Minus album (before they went cock-rock), I liked the way distortion was used. Also, as already mentioned:
