What's the stupidest question you've ever been asked on an Official Document?

I just filled out a job application that included the following beauty:

"Are you a member of any secret organisations, eg the Freemasons?"

Yes, because obviously if I was a member of such a SECRET society, I would disclose this SECRET on a job application. I dunno, maybe there's a feared Illuminati / Freemason / Opus Dei takeover of local Government in the UK...
