I don't like fees, so...

In the "first name" box are you supposed to put a middle name as well if your registered name for your bank account includes one? It only asks for first name singular... but wants the name exactly as it is on the statement, so I'm confused. And google isn't helping me. And I don't want to do it wrong and get charged. Thanks, etc.
