Right to die cards / living wills / how do I do this?

So that's a one council thing, or whatev. And obviously people are saying that in an accident the hospital are hardly going to be searching through your bag to find one. Well, anyhow, say you had one of these things hung around your neck, and were then in a car accident, or suffered some kind of very serious accident based injury, that would almost certainly result in severe physical or mental disability. Would they work for that? As in the hospital wouldn't resusitate or provide life-extending treatment? Does that make sense? I don't know. Or are these things and living wills only supposed to provide for illness? Cause I wouldn't mind punching a hole in one of those things and wearing underneath my clothes in a necklace thing if that would work. I worry about all this so much, I always have, surviving an accident/illness with mental problems in particular terrifies me more than anything I could possibly think of.
And if you can do something along that lines with a living will, how does it work? I looked on the internet but it was too confusing, saying you had to be specific, but could write in laymans terms etc. I mean, could you say that you want to refuse any ressusating / life-prolonging / other treatments that would likely result in you surviving with serious physical / mental disabilites? I mean, obviously in immediate terms it wouldn't be known about, but a lot of these decisions are made later on, and if family knew about it would point it out. My sentences are getting so bad. Or does it have to be ultra-serious, i.e. if you'd be brain dead or something? Has anyone done one of these?
